Thyroid Function Test (TSH, T3, T4)

What is a Thyroid Function Test (TSH, T3, T4)?

Thyroid function tests measure the hormones that control your body's pace. TSH is the main screening test.

Why this test is done

  • Detect an underactive thyroid (hypothyroidism) or an overactive one (hyperthyroidism)
  • Monitor and adjust thyroid medicine doses
  • Investigate tiredness, weight change, hair fall, irregular periods or infertility
  • Routine testing in pregnancy

How to prepare

  • No fasting is required for a thyroid test
  • If you take levothyroxine, take the sample before your morning dose unless told otherwise
  • Tell the laboratory and your doctor about all medicines and supplements, including biotin, which can interfere with results

Understanding your results

  • A high TSH with a low free T4 indicates hypothyroidism — the thyroid is underactive
  • A low TSH with a high free T4 indicates hyperthyroidism — the thyroid is overactive
  • A mildly raised TSH with a normal T4 is called subclinical hypothyroidism and is not always treated; your doctor decides based on your symptoms and other factors
  • Reference ranges differ in pregnancy

Read your report with your doctor

Reference ranges differ between laboratories, and a value slightly outside the range is often normal for you. What matters is the pattern across your results, your symptoms and your history — which is why the report belongs in a consultation, not a search box.

Frequently Asked Questions

Do I need to fast for a thyroid test?

No, fasting is not required. If you already take thyroid medicine, it is usually best to give the sample before your morning dose so the reading reflects your baseline.

What does a high TSH mean?

A high TSH with a low free T4 indicates an underactive thyroid, or hypothyroidism. A mildly raised TSH with a normal T4 is called subclinical hypothyroidism and is not always treated — your doctor decides based on your symptoms and other factors.

Should I take my levothyroxine before a thyroid test?

Usually the sample is taken before your morning dose, so the reading reflects your baseline rather than the peak just after a tablet. Follow whatever your own doctor has told you, and tell the laboratory when you last took it.

How do I check for low thyroid?

With a blood test. TSH is the main screening test, usually done together with free T4. No fasting is needed. A high TSH with a low free T4 indicates an underactive thyroid. Symptoms such as tiredness, weight gain, feeling cold, dry skin and hair fall may prompt the test, but they overlap with many other conditions, so the diagnosis is made on the blood result rather than on symptoms.

My TSH is low — what does that mean?

This is the part that catches people out. In everyday speech 'low thyroid' means an underactive one, but a LOW TSH on your report usually points the other way — to an overactive thyroid, or hyperthyroidism. TSH is the signal your brain sends telling the thyroid to work harder. When the thyroid is already producing too much, the brain stops sending the signal, so TSH falls. A HIGH TSH is what suggests an underactive thyroid. Either way the result is read together with free T4, and by a doctor, not from the printed range alone.

How do I know if my thyroid is underactive or overactive?

From the blood test, not the symptoms — they overlap too much. High TSH with low free T4 means underactive. Low TSH with high free T4 means overactive. Underactive tends to cause tiredness, weight gain, feeling cold, dry skin and constipation; overactive tends to cause weight loss, feeling hot, a racing heart, shakiness and anxiety. But plenty of people have some of both, which is why the test decides it.

What this costs, and what is free

Where it is free

Routine blood tests are generally free at government hospitals and health and wellness centres when ordered there.

If you go private

Private lab prices for the same test vary widely between cities and between labs in the same city. It is worth asking the price before agreeing, and worth asking your doctor which tests are genuinely needed rather than accepting a bundled package.
